Hi. There is a cross-over hose between the two heads at their tops (hard to see in your pic). And the port/left head connector has a second hose connected that goes to the tell-tale spout (pisser) on the boat hull. These thru-hull spouts often get clogged by bugs. This hose must be clear. Blow some compressed air into the spout to ensure it is not clogged. When running on-the-hose or in the water, this hose lets air escape the heads while they fill up with water... and then will stream out water showing proper cooling flow.
