If you're talking about the stem, it could just be from old age especially if they're the original ones from 96.

Are we talking about the threaded top that threads onto the actual valve, or the mushroom top with the red inner part?

It could have been from over tightening but more times than not it is because the valve and housing are worn allowing hot gasses in and it actually melts the threads out.

They also wouldn't have been painted like that from the factory so somebody has been in that engine.

If they are the good strong zip ties with the stainless locking tab they are fine to use.
Also make sure the clamps are all stainless as some of the cheap ones have a steel screw and will rust out.

I tried replacing just the threaded cap but it has done it again. So I understand I need to replace that valve. The left valve is good. Why is one good and the other keeps sending hot gasses?

O-ring could be in worse shape or even gone in one and not the other. Take them apart and inspect the holes in the housing that the stem of the valve slides through. I've notice when excessively worn they are usually worn in a slight oval shape. Even if not there should be no slop in the fit and the o-rings need to be in good shape.

Had the same problem with mine. Melted two of them. My housings and stems were wornI bought a good used set from minnetonka4me. Problem solved.
