Notice something didn't feel right when pulling away with the trailer almost as if it wasn't latched. When I looked underneath at the piece that would hold on to the ball I noticed that the nut holding it in place was almost completely unscrewed.
So now what I do is tighten the nut down properly and then deliberately damaged a few of the threads just in case to not does every back off again it will stop.
